A short guide for patients

1. Using an Internet Browser go to https://www.iaptportal.co.uk. You should have your SecureID #1 and SecureID #2 at hand. These will be provided to you by the service.

Footer: A note reminding users to contact their service if they don't know their Secure IDs. The bottom blue bar credits PCMIS with a 2026 copyright.

2. Enter your ‘SecureID #1’ on the first page and select ‘Next’. IMPORTANT NOTE | If your local service use PCMIS Patient Portal SMS feature and you have consented to be contacting by mobile phone, you will receive a text message containing a unique URL link. Clicking this link will skip Step 1 and take you straight to the Portal log in screen, where you will only be required to enter your SecureID #2 and Date of Birth before completing your assessments.

Service Name: Identifies the specific service as "York PCMIS" (York PCT IAPT Service). Logos: Features the PCMIS logo and the official NHS logo. Input Fields: Secure ID #2: Shows an example numeric ID (298289166). Date of Birth: Shows a placeholder date (29/10/1966). Action: A green "Login" button. Details: Notes that questionnaires take 15–20 minutes. There are links to download the app on the App Store and Google Play. Support: Includes a contact number (01904 321322) and a "donotreply" email address.

3. Enter your ‘SecureID #2’ and ‘Date of Birth’ by either manually typing it in (e.g. 25/10/1980) or selecting the date from the pop calendar button and click ‘Login’.

4. You will be guided through each assessment in turn. Please complete the assessments by selecting the answer that relates most to your circumstances for each question.

An example of the assessments is shown below but please don’t worry if your screen looks different. These can differ depending on what assessments the service would like you to complete.

Trouble falling or staying asleep OR sleeping too much? Options: Four radio buttons ranging from "Not at all" to "Nearly every day."

If you make a mistake then select the ‘Clear Form’ button to clear all the answers entered on that assessment or ‘Next’ once the assessment is complete, to navigate to the next assessment.

Header: "Client Portal." Navigation Tabs: Shows a longer list of completed assessments (including GAD7, K10, CDRISC25, OCI-R, PCL-C, SCS, and ACE). Status: A green tab labeled "Completed" is highlighted with a checkmark. Main Message: "Assessments completed. Thank you, all the questionnaires have been been completed for the day." Footer: Standard links for Legal Statements, Privacy, Security, and About.

5. Once all assessments have been completed simply close the browser tab or move to another page.

Notes:

  • You are required to complete all questions on the current assessment before you can proceed to the next assessment.
  • If for any reason the Internet session is interrupted or the web browser experiences any problems, you can log back in to the Patient Portal and complete the remaining questionnaires. Any questionnaires that were completed in the original session will have been saved.
  • It is only possible to complete the assessments once each day. If you login and try to complete the assessments twice in the same day you will get a message on the screen advising that the assessments have already been completed.
  • The Patient Portal Web Application is compatible with a range of Internet Browsers and mobile devices including Microsoft Edge, Google Chrome, Mozilla Firebox and the iPad Safari browser.
